Referrals

We welcome referrals from physicians (MDs), nurse practitioners (NPs), naturopathic doctors (NDs), physiotherapists, and other regulated healthcare providers. We offer timely access with minimal wait times, and our Patient Care Coordinator will contact referred patients directly to arrange their appointment.

Pain Management & Injection Referral Form
For patients being referred for assessment and consideration of ultrasound-guided injections or other interventional pain-management procedures.

Iron Infusion Referral Form
For physicians, nurse practitioners, and naturopathic doctors referring a patient for intravenous iron therapy. Please include recent laboratory results, relevant medical history, and the requested iron formulation and dose, when specified.

Submission of a referral does not guarantee treatment. Each patient is independently assessed to confirm that the requested service is appropriate and can be provided safely within our clinic.
