Your intake form must be completed before your appointment. Please ensure your health history, medications, supplements, and allergies are up to date. If you need assistance, contact our team before your visit.

A valid credit card is required to reserve your appointment. Please add your card through your secure patient account within 24 hours of booking. Appointments without a card on file will be released.

We require a minimum of 24 hours’ notice to cancel or reschedule. Appointments cancelled or rescheduled with less than 24 hours’ notice, along with missed appointments, will be charged the full appointment fee to the card on file

We offer direct billing to many extended health insurers. Your insurance coverage is an agreement between you and your insurer. Please confirm your eligibility, benefit limits, and coverage before your appointment.

Clarity Wellness Clinic cannot guarantee reimbursement. If your plan does not support direct billing or your claim cannot be processed, full payment is required at your visit. You may submit your receipt to your insurer for reimbursement according to your plan. Our services are not covered by MSP.

Arriving on time helps you make the most of your visit. If you arrive late, we may need to shorten your appointment to keep subsequent visits on schedule. The full appointment fee will still apply.
